In the past week, Amazon experienced 4 Sev 1 incidents - this is when critical systems go down. One of them blocked users from accessing their accounts, purchases, and prices for 6 hours. The cause was a deployment of code generated by AI tools.

Previously, there was a case with their AI tool Kiro, which deleted the environment it was operating in - a 13-hour outage of AWS Cost Explorer. Amazon stated that it was a "user error, not AI" - implying that the employee had overly broad access rights.

An internal brief for the meeting describes a "trend of incidents" over the past months with a "high blast radius" and "Gen-AI assisted changes." Now AI-generated code will need to be approved by senior engineers before deployment.
